Teen of the Month Application

So, you want to be famous? Here is your chance!

A Teen of the Month is someone who stands out in his/her Teens in the Driver Seat® group. He/She motivates other Teens in the Driver Seat® members, and spreads the safe teen driving message whenever and however possible.

To be considered as a Teen of the Month, you must:

Be a junior high or high school student at a program school (currently have a TDS team),
Be involved in a Teens in the Driver Seat® program, and
We must have a signed video/photo release form on record (and if you’re under 18, have your parent sign it). You can fax this to 210-979-9694 or email a good picture (with your phone) or scan.

Submit your application to be the Teens in the Driver Seat® Teen of the Month. Just fill out the stuff below and submit it.

Be as clear and expressive as possible. 1 sentence and "because I have to for a class or requirement" answers will not be selected. We want to know why you care about teen traffic safety.
If you could change one thing about the way your friends drive or ride in the car, what would it be?*

We need a photo release form for you (link above in instructions). We cannot select you as a Teen of the Month without it. If you've turned in this form before, you're set. If you aren't sure, email TDSWebmaster@tti.tamu.edu with your full name and school to ask!
